We are looking for an Intake RN that could be located in either Wausau WI or Calumet MI. The Registered Nurse - Intake is responsible for receiving/processing referrals and ensuring that hand off for admission occurs in a timely and consistent manner. The Intake Nurse enters referral information into NetSmart to facilitate the start of care. This person frequently interacts with referral sources, other members of the referral center staff, all branch locations and all others necessary to facilitate the start of patient care. Utilizes and promotes use of excellent customer service skills when working with internal and external customers. Will be expected to use critical thinking skills to resolve and prioritize activities. Is responsible for taking and processing verbal orders within scope of practice. Referral source interactions include, but not limited to, working closely with referring hospitals, physicians, board and care facilities, skilled nursing facilities, patients, families, and the general public. Job Specific Duties · Responds to communication regarding potential referrals for services with an extremely customer friendly, service orientation at all times. · Accurately completes referral documentation as necessary to facilitate the admission process. · Processes and documents referrals that are not accepted for admission, indicating specific reason for non-acceptance. · Reviews home care order and contacts referral source for clarification of orders when necessary. Assigns diagnosis codes to admissions as appropriate and/or validates diagnosis codes identified by clerical staff. · Attends department meetings and serves on committees and project teams as requested. · Participates in the development of policies and procedures and actively assists in problem solving with and between departments to ensure the intake function is collaborative and effective. · Accepts clinical assignments as designated by the manager. · Maintains positive professional relations with support personnel through appropriate and timely responses for information or paperwork necessary for the key clinical and billing functions for the agency. Experience · RN: Minimum of six months recent clinical experience required, with a minimum of one year preferred. · Experience in coordinating/processing referrals and/or at least one year's experience in a home health, hospice, or community health setting strongly preferred. Required · Possesses an active Wisconsin and Michigan RN nursing license within 90 days of hire. Other · Must have the clinical knowledge and critical thinking ability to effectively plan and provide coordination of patient care consistent with the standards and regulations. · Must have exceptional interpersonal and customer service skills. · Must be able to effectively solve unique problems as they arise or identify when to consult the supervisor. · Must have knowledge of current home health and hospice administration criteria. · Must be able to demonstrate problem-solving abilities and have excellent telephone, interpersonal and written communication skills. · Strong commitment to teamwork and customer service, both internally and externally, is required. · Must have strong/proven data entry and general computer skills.
