Volunteers of America, The Homestead at Rochester is looking for an experienced Licensed Practical Nurses (LPN) to join its team. The LPN role shall perform in accordance with state and local laws and within the guidelines of his/her professional organization. Our person centered care approach provides a low nurse to patient ratio, which allows you to make a real difference in the lives of those in your community. You will truly provide exceptional cares and clinical outcomes to your residents. CORE RESPONSIBILITIES: * Observe residents physical and psychosocial status, assign personnel appropriately to meet resident needs. * Ensure unit personnel and volunteers carry out assignments * Maintain communication with supervisor and staff * Document and report all incidents * Perform nursing techniques for the comfort and well-being of the resident * Administer prescribed medications and treatments per MD orders * Participate in resident care plans as directed What We Do : Our campus offers a full continuum of health care including Assisted Living, Skilled Nursing, Specialized Memory Support, Home Health and Outpatient Services More Than a Job: Working here- our team has the opportunity to serve others. Job Requirements QUALIFICATIONS: To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. Graduate from an accredited school of nursing with current licensure as a Licensed Practical Nurse in the state of practice. 2. Effective communicator with staff, families and outside agencies. 3. Ability to understand residents' charts, doctor's orders, residents' plan of care, medication orders. 6. Must posses the ability to effectively gather clinical data in regards to resident status and condition. 7.
