Sinai Health System Staff Registered Nurse - Oncology Nurse Navigator in Chicago, Illinois

The Oncology Nurse Navigator is a professional nurse who functions as a clinical resource to the Oncology Program, coordinates referrals from physicians and health care facilities for patients, guides patients through the health care system by assisting them with access issues and provides a significant level of patient education related to oncology diagnosis and planned treatment.

Job Summary:

  • Initiate communication with patient upon learning they have an abnormal/suspicious radiology exam or positive biopsy

  • Assess and document the need and concern of patient and family individualizing care

  • Guides oncology patients through the health care system and help patients arrive at scheduled appointments on time and prepared.

  • Inform patients and primary care physicians of test results

  • Assist patients in access to services and programs throughout the cancer care continuum; assists with setting up transportation if needed.

  • Facilitates patient interaction and communication with health care staff and providers

  • Responsible for Insurance Verification and precertification for Oncology patients. This includes but is not limited to initiating external referrals in an effort to assist our patient’s.

  • Responsible for verifying patient demographics, including next of kin and emergency contact with each encounter.

  • Promotes adherence to Standards of Nursing Practice by caregivers

  • Conducts needs assessment and develop individualized educational plan for patient and families

  • Educate patients and families about the cancer disease, and treatment process through provision of oral or written resources.

MINIMUM Education:

  • BSn

MINIMUM WORK EXPERIENCE:

  • Minimum of 2 years Oncology experience

KNOWLEDGE & SKILLS:

  • In-depth knowledge of cancer care, including both clinical and supportive care

  • Knowledge of community based cancer support services

  • Excellent verbal, and written communication skills

  • Bilingual in Spanish preferred

  • Proficient in Microsoft Office

REQUIRED LICENSES, Certificates, Registrations:

  • Current licensure in the State of Illinois as a Registered Nurse

  • CPR certification

  • Oncology Nursing Certification preferred
