We are looking for hard working, honorable caregivers to provide supporttothose who need some extra help in their homes.

Home Setting:

  • Under the supervision of a registered nurse, provides basic care to clients in their home to maintain optimal physical and emotional comforts.

  • Assists in providing and/or maintaining optimal physical and emotional comforts to clients and their families.

  • Provides clients with personal care and activities to include bathing, oral hygiene, skin care, toileting, hydrating, feeding, ambulating, changing of bed linens and repositioning.

  • Assists clients and their families toward achieving maximum self-reliance within their environment.

  • Turns client in bed; transfers client between bed, chair, and commode.

  • Prepares meals, including those requiring special diets when indicated.

  • May accompany client to the physician's office or clinic, etc..

  • May perform basic non-sterile treatments and procedures under the supervision of the registered nurse.

  • May assist clients with exercise and range of motion according to the care plan developed by the registered nurse/therapist.

  • May remind clients to self- administer medications.

  • Keeps registered nurse informed of the client's progress and reports all changes in the client's condition. Receives and gives report on assigned client with other shifts as appropriate.

  • Submits clinical documentation on a timely and accurate basis.

  • Participates in in-service and orientation programs and other required meetings.

  • Demonstrates a commitment to maximizing the satisfaction level of all internal and external customers through utilization of quality improvement principles.

  • Maintains a safe work environment and practices personal safety techniques to prevent injury to self and others.

  • Performs other related duties as required.

PRINCIPLE DUTIES AND RESPONSIBILITIES:

Receives report from previous nurse aide as well as from the registered nurse assigned to the client. Provides constant observation of the client. Reports all unusual, unsafe behavior or alteration in physical condition immediately to the registered nurse.

Assists in providing the client’s personal care and activities to include bathing, oral hygiene, skin care, toileting, hydrating, feeding, ambulating, changing of bed linens and repositioning.

Signs in and out at the beginning and end of the shift following the designated process in each facility.

Remains with the client during the entire shift until scheduled relief arrives.

Provides assigned nurse or charge nurse with an evaluation form upon completion of shift.

Submits clinical documentation accurately and in a timely manner.

Assists in the optimal functioning of clients and their families in coping with their environment through the provision of the support services of personal care and homemaking activities.

Turns client in bed, transfers client from bed to chair, from chair to commode and vice versa.

Assists client in maintaining a clean, healthful and pleasant environment.

Prepares meals, including those requiring special diets, when indicated.

May accompany client to the physician’s office or clinic, etc.

May perform basic non-sterile treatments and procedures under the supervision of the registered nurse.

Keeps registered nurse informed of the client’s progress and reports all changes in the client’s condition.

Receives and gives report to other shifts on assigned client as appropriate.

Participates in inservice and orientation programs and other required meetings.

Demonstrates a commitment to maximizing the satisfaction level of all internal and external customers through utilization of quality improvement principles.

Promotes and demonstrates respect for cultural diversity through daily interactions with clients, co-workers and all internal and external customers.

Performs other related duties as required.

EDUCATION & EXPERIENCE:

  • Requires a high school diploma or GED.

  • Requires at least one year of recent hospital, nursing home or home health care experience.

  • Ability to pass the NLN Skills Assessment Test with a score of 75% or better.

  • Ability to pass Technical Skills Checklist. Completion of a certified nurse aide training program is preferred with one year of recent hospital, nursing home, or home health care experience.

  • BCLS certification is preferred.

  • Must possess a current State of Michigan drivers license and a satisfactory driving record.

  • Must have access to reliable transportation and a telephone.

  • Must be able to transport or move client; carry, bend stoop, climb or otherwise be physically active in the course of job duties.
