This is a part time 0.5 FTE (20 hours per week) position and is eligible for benefits.
We are seeking a Registered Nurse - Clinical - Behavioral Medicine to coordinate care for Pacific Medical Centers’ Managed Care patient in order to assure cost effective, appropriate utilization of health care resource.
In this position you will:
Collaborate with physicians to identify the plan of care for admission, including the expected outcome(s) of treatment.
Assess patients and facilitate a decision to determine disposition of patients on discharge.
Work with chemical dependency, residential treatment, and eating disorders treatment facilities’ discharge planners to maximize outcomes.
Advise patients and families regarding benefits, resources, and home management options.
Be accountable for appropriate referrals for homecare.
Assess and determine homecare for patients when hospitalized.
Recommend care plan for high risk patients in the outpatient setting including communication with physicians, nurses, homecare providers or others as applicable.
Collaborate with the physician or provider regarding appropriate disposition of patients from hospital; educate and direct providers both within and outside the Pacific Medical system regarding benefits and utilization of services; direct case managers from outside facilities regarding: use of benefits and discharge planning options; consult with USFHP Medical Director regarding complex cases and discusses appropriateness of hospitalization or care in treatment facilities; identifies issues with Pacific Medical system to facilitate patient care (barriers to patient care).
Required qualifications for this position include:
Current Washington State Registered Nurse License.
Five (5) plus year’s clinical experience.
Two (2) plus year’s in a Behavioral Health setting.
CPR or BLS certification within six (6) months of hire.
Preferred qualifications for this position include:
Certified Case Manager.
Previous experience in utilization review/case management.
Ability to use an Electronic Medical Records (EMR) system.Bilingual.
