Job Summary

Responsible for coordination of an interdisciplinary team approach in the provision of skilled nursing assessment, planning and care in order to maximize the comfort and health of patients and families consistent with the policies and procedures of Lourdes HomeCare.

Develops a holistic, multidisciplinary plan of care utilizing the job functions within each of the five core components of case management: Coordination and Service Delivery; Physical and Psychological Factors; Benefit Systems and Cost Benefit Analysis; Case Management Concepts; and Community Resources. Interacts with Care Management Team and physicians to determine appropriate Home Care/Hospice Service for patients/clients based on regulatory guidelines for each line of service.

Qualifications - Minimum

Associate Degree in Nursing (ADN) from an accredited school of nursing

Kentucky Registered Nurse License; or active compact state-issued license

Basic Life Support (BLS) through AHA

Two years of experience in acute care

Qualifications - Preferred

Bachelor of Science in Nursing (BSN) from an accredited school of nursing

One year of home care experience

Physical Demands

Works within patient care units and other areas as assigned. Working beyond normal working hours, on weekends, and holidays, and in other positions temporarily, when necessary. Working throughout the facility moving around intermittently thoughout the shift. Frequent interruptions.


8:00 am - 4:30 pm

Shift and Job Schedule

Day shift; rotating schedule.
