CHI Health at Home is looking for Full-time Nurse Practitioner for our Hospice division who are compassionately committed to providing quality care and service and have at least one year of clinical experience.

Responsibilities

  • Provide face-to-face assessment services to hospice patients to assist the Hospice Medical Director and Hospice Team to determine each patient’s continuing eligibility for the hospice program.

  • May provide service in various settings, including the patient’s home, a physician’s office, the impatient hospice facility, etc.

Qualifications

  • Graduate of an accredited school of nursing.

  • Currently licensed to practice in Nebraska, with Masters in Nursing.

  • One year of nursing experience required.

  • One year of hospital nursing experience preferred.

  • Must obtain and maintain current CPR Certification.

  • Valid Driver’s/Chauffeurs license and auto liability insurance coverage according to CHI Health at Home policy is required for travel.

  • Compliant with appropriate state certification/registration and CHI Health at Home requirements as necessary.
