Type Full-Time Location US-MN-Minneapolis/St. Paul Overview: Ecumen Hospice is looking for a full time Registered Nurse (RN) Case Manager to join its growing, dynamic team providing services in Chisago County and surounding communities! The Ecumen Hospice team's mission is to provide care that exceeds the expectations of those that find themselves in the later stages of life by providing a level of comfort and quality of life that makes each person feel honored and supported as their life continues to have significance. The Hospice office is located in Oakdale, Minnesota. Responsibilities: The RN Case Manager is tasked with the responsibilities of: delivery of patient care, supervision of hospice aides, education of patients and their caregivers, and participation in quality assurance of patient care initiatives. Specific responsibilities include: Completes an initial, comprehensive and ongoing comprehensive assessment of patient and family to determine hospice needs. Provides a complete physical assessment and history of current and previous illness. Assesses and evaluates patient's status by writing and initiating plan of care, regularly re-evaluating patient and family/caregiver needs as well as participating in revising the plan of care as necessary Utilizes health assessment data to determine nursing diagnosis. Develops a care plan that establishes goals, based on nursing diagnosis and incorporates palliative nursing actions. Includes the patient and the family in the planning process. Initiates appropriate preventive nursing procedures. Administers medications and treatments as prescribed by the physician in the physician's plan of care. Counsels the patient and family in meeting nursing and related needs. Qualifications: This position requires a person who is a graduate of an accredited school of professional nursing. One or two years of nursing experience in post acute care setting is required. One to two years of nursing experience in Medicare Hospice is preferred.
