Atlantic Health System Nurse Case Manager in MORRISTOWN, New Jersey

Atlantic Health System is seeking a FT Nurse Case Manager for the Atlantic MSO unit working primarily 9:00AM - 5:00PM.

Essential Job Functions:

  • Oversees care coordination for assigned ACO beneficiaries to ensure continuity of care and overall beneficiary needs are met.

  • Appropriately supports ACO population health initiatives in concordance with ACO goals and beneficiary outcomes.

  • Supports the integrity of the care continuum by collaborating with a multidisciplinary team, which thru effective communication will ensure the plan of care.

  • Tracks and collects all data related to the transition of care.

  • Collaborates and communicates appropriate data findings to Primary Care provider, Care Coordination Team, as well as healthcare providers across various health care settings.

  • Utilizes excellent writing skills, and accurate and clear communication to relay findings to healthcare team.

  • Using Transitional Care Nursing guidelines, assesses/identifies the needs of the individual/family/caregiver.

  • Coordinates care for high risk patients and provides pathway for smooth transition back to the community.

  • Completes medication review, makes referrals to appropriate community resources, and follow-up with PCP.

  • Documents actions in a timely manner, communicates findings to the primary care provider and/or multidisciplinary healthcare team.

Education:

  • Graduate of and accredited Nursing Program. BSN required.

  • CCM/CCTM a plus.

Required License:

  • Current NJ nursing license required. Current PA and NY nursing license a plus.

Related Experience:

  • Minimum of 5 years of clinical experience as a Registered Nurse. Willingness to travel and flexible work schedule a must.

  • Community health experience and knowledge of community healthcare resources preferred.

  • Strong clinical, interpersonal, analytical, priority setting, problem solving, and organizational skills.

  • Ability to work with minimal direct supervision.

  • Ability to navigate EHR and data bases with demonstrated ability for electronic clinical documentation.

  • Clear and accurate verbal, telephonic, and written communication skills.
