JOB REQUIREMENTS: Hospice Nurse Practitioner Job Code: 2017-HHU:ME-008 * Conducts face-to-face encounter visits to assist the Hospice Medical Director in determining eligibility/continued eligibility for hospice care. Completes a narrative and signs and dates the attestation verifying completion of the face-to-face encounter. Provides clinical findings from the face-to-face encounter to the Hospice Medical Director. * Works collaboratively with the Hospice Medical Director, and Patient Care Manager. Consults with the Medical Director and primary physician of services on patient care. * Communicates with and updates PCM of activities and issues within branch on a regular basis including complaints. * Provides oversight of best practices related to symptom management including pharmaceutical oversight. * Makes visits to hospice patients and provides skilled nursing care within the established plan of care. Performs clinical care tasks including evaluation of patients and physical examinations. Updates the plan of care as needed. Performs case management. Contacts and consults with the physician regarding the plan of care. Communicates significant changes in patient condition. * Provides information, instruction and education to the patient, patient family members and caregivers. * Documents visit notes, treatments, and care. Records and reports symptoms and changes in patient's condition. Completes and transfers data on a timely basis. Verifies that physician orders for all disciplines are complete, accurate and cover the care and visits provided. Ensures orders are obtained prior to service delivery. Documents any verbal orders and follows up to secure written orders. Follows visit pattern according to physicians orders and ensures required visits are completed in a timely manner. * Reviews and analyzes outcome reports for the area in collaboration with PCM. Monitors quality measures. * Serves as a resource and provides education to hospice staff on relevant topics related to team member roles and hospice practice. Provides training on documentation, nursing skills, etc. * Attends and participates in mandatory IDG meetings, in-services, and conferences. * Completes and submits timesheets or time logs to supervisor on a timely basis which accurately document hours, mileage, travel and other time worked. * Assists with audits as requested.
