Visit: The Care manager is involved in the coordination of care andservices for patients admitted to Good Samaritan Medical Center who areidentified as needing assistance or meeting Care Management criteria. The CareManager is responsible for utilization review, utilization management,care coordination, and discharge planning. Information is gathered via anin-depth assessment that provides the healthcare team with the patients priorlevel of functioning, access to and/or use of community resources andavailable support systems. * Assesses for appropriateness of level of care setting from admissionthrough discharge. * Identifies patients requiring care management and takes the lead ascare manager for those requiring intervention, especially with clinicallycomplex cases * Conduct on-site review of medical records at clinical facilitiesaccording to organization standards. * Maintain frequent telephone contact with clinical service providers toperform defined duties and facilitate communication between the organizationand the provider. * Document UR/case management activities such as precertifications,concurrent and retrospective reviews. * Consult regularly with other organization staff to review treatmentplans when level of clinical care requires further clarification. * Maintain a tracking system and clear, concise records for monitoringand reviewing cases. * Prepare documentation/case synopsis upon closure of cases asrequired by clients. * Provide phone crisis intervention services and precertificationevaluations for inpatient hospitalization and other levels of care. * Provide consultation and training to providers regarding authorizationprocedures and utilization review for a specific course or nature oftreatment. * Contribute to policy review and procedure development and evaluation. * Performs other duties as assigned, some of which may be essential tothe job. * Graduate of an accredited school of nursing * BSN required within 5 years of employment * Minimum of 3 years of clinical nursing experience, required * Knowledge and experience with managed care, requiredSCL Health is an Equal Opportunity Employer.
