The Registered Nurse BSS provides direct nursing care in a diverse healthcare environment in accordance with established policies, procedures and protocols of the healthcare organization.
Implements and monitors patient care plans. Monitors, records and communicates patient condition as appropriate.
Serves as a primary coordinator of all disciplines for well-coordinated patient care.
Notes and carries out physician and nursing orders.
Assesses and coordinates patient's discharge planning needs with members of the healthcare team.
Performs professional nursing activities according to the nursing care plan and in collaboration with the care manager.
Conducts comprehensive nursing assessment at time of admission. Monitors continuing status and identifies significant charges which impact care.
Accurately documents and communicates assessment data, plan of care and nursing intervention within established agency standards. Organizes and analyzes assessment data to synthesize nursing diagnosis(es). Prioritizes nursing care needs.
Administer pneumonia & influenza immunizations at community-based clinics. Provide good community relations between Borgess Health and the community.
Develops and documents nursing care plan. Incorporates nursing outcomes into plan of care. Implements nursing care plan. Coordinates client care.
Demonstrates sound judgment in the practice of nursing. Evaluates outcomes of activities performed during visit or shift if doing facility staffing. Evaluation of data is used to revise diagnosis, outcomes and plan of care, as needed.
Conducts nursing visits in an efficient, cost effective manner. Maintains daily visit quantity within established agency productivity standards if applicable.
Supports agency mission, vision and goals. Conducts self in professional manner. Follows agency policies and procedures.
Maintains appropriate professional appearance. Assumes responsibility for professional development. Contributes to the professional growth of others.
Participates as a team member utilizing a collaborative style (nursing team, interdisciplinary team, intra/inter agency communications). Directs and evaluates care delivered by HHA's when delegated to do so.
