Kalamazoo Community Mental Health Integrated Healthcare Nurse/Care Manager (RN) in Kalamazoo, Michigan

Integrated Healthcare Nurse/Care Manager Kalamazoo Community Mental Health and Substance Abuse Services is seeking an Integrated Healthcare Nurse/Care Manager who has experience working with individuals of diverse racial, ethnic, and cultural backgrounds. The successful candidate will: • Provide care management functions in collaboration with primary care sites. • Ensure the provision of population management services, including identifying with a primary care physician individuals with high medical and psychiatric co-morbidities with poor coordination. • Develop, implement, and evaluate individualized patient care plans. • Enhance communication and collaborative relationships with multidisciplinary healthcare team members. • Serve as a liaison between patients, their families, and healthcare providers. • Be responsible for monitoring service outcomes. • Be responsible for the implementation of effective UM strategies. • Analyze data to improve provider performance and member satisfaction. • Provide enhanced care coordination services for a caseload of 110-125 high-risk patients/consumers. • Triage referrals, schedule mental health assessments, services, and consultations. • Facilitate joint system case consultations and treatment planning for individuals with high-risk co-morbid conditions. • Assist with transitional care.Specialized Training, Skills, Abilities, Experience: • Associates degree or higher in Nursing, Applied Science or related field. • Minimum of 2 years experience in primary and/or behavioral health care services. • Strong understanding of the needs of persons with mental illness, intellectual/developmental disabilities and substance use disorders, and co-morbid primary care conditions; understands how to implement a strong recovery philosophy. • Advanced computer skills, including word processing, spreadsheets, database management and e-mail. • Ability to develop and collaborate with teams, excellent communications skills, interpersonal and human relations skills. • Lived experiences with behavioral health issues are desired. • Required license: Registered Nurse license (RN) in the State of Michigan and a valid Michigan Motor Vehicle Operator's license. Use of video display terminal (VDT) for periods in excess of 30 minutes at a time. Possible eyestrain from extended periods of viewing VDT. May be exposed to bloodborne pathogens, infectious diseases, and parasites. Travel throughout the Kalamazoo area is required.
