Responsible for providing quality education, follow-up and emotional support to the population and their families. Case manages the population throughout the inpatient and outpatient settings and measures outcomes to maximize continuity of care between the settings. Manages projects and programs related to clinical management, performing quality improvement activities to measure program processes and outcomes.

Other job responsibilities and duties include:

  • Navigating patients through a disease process from a multidisciplinary approach.

The nurse navigator’s role is to prepare for the patients’ attendance in the clinic, coordinate the clinic during the appointment time, and implement all proposed recommendations. Often times 4 to 5 patients attend multidisciplinary appointments at a time. The team made up of a neurologist and therapist (OT, PT, SLP, SW, RDN, Pharmacist).

  • Act as a point person for Spectrum Health’s ALS and Parkinson’s Disease patient population.

  • Must be a strong Patient advocate.

  • The nurse navigator assists neuromuscular and movement disorder neurologists with program development locally, regionally, and nationally.

  • Organize regular support groups for both ALS and Parkinson’s Disease.

*Basic Qualifications:


Education - Bachelor's Degree or equivalent in Nursing

Experience - 2 years of experience typically gained through skills/knowledge/abilities in previous nursing or CDE clinical experience

Licenses - Registered Nurse (RN) Upon Hire

Basic Life Support (BLS) Upon Hire

Preferred Qualifications:

Education - Master's Degree or equivalent in Nursing

Licenses - CDE certification

Previous experience with patients within the neuromuscular population and movement disorder neurology

Experience with insurance guidelines in the ambulatory care setting is helpful
