The Registered Nurse, Case Manager shall perform a full range of certified nursing services. The Certified Nurse, Case Manager shall meet or exceed the industry standards in accordance with established principles, practices and ethics of a Registered Nurse. The Defense Health Agency (DHA) is a joint, integrated Combat Support Agency that enables the Army, Navy, and Air Force medical services to provide a medically ready force and ready medical force to Combatant Commands in both peacetime and wartime. The DHA supports the delivery of integrated, affordable, and high quality health services to Military Health System (MHS) beneficiaries and is responsible for driving greater integration of clinical and business processes across the MHS.

All services shall be provided in accordance with established standards, principles and ethics of the profession, applicable professional specialty organizations, and the high-quality standard for which CHS is recognized.


This position is based Travis Air Force Base, CA.


Perform and/or provide direction and support for the following services:

  • Participate in all phases of the Case Management Program (CMP), and ensure that the CMP meets established case management (CM) standards of care.

  • Provide nursing expertise about the case management process, including assessment, planning, implementation, coordination, and monitoring.

  • Recognize case management process opportunities while identifying and integrating local case management processes.

  • Develop and implement local strategies using inpatient, outpatient, onsite and telephonic case management.

  • Establish and implement tools to support case management, such as those used for patient identification and patient assessment, clinical practice guidelines, algorithms, case management software, and databases for community resources.

  • Integrate case management and Utilization Management (UM) while integrating nursing case management with social work case management.

  • Maintain liaison with appropriate community agencies and organizations.

  • Accurately collect and document patient care data.

  • Develop treatment plans including preventive, therapeutic, rehabilitative, psychosocial, and clinical interventions to ensure continuity of care toward the goal of optimal wellness.

  • Establish mechanisms to ensure proper implementation of patient treatment plans, and follow-up post discharge in ambulatory and community health care settings.

  • Provide appropriate health care instruction to patient and/or caregivers based on identified learning needs.

Required Skills


  • Bachelor’s Degree of Science in Nursing.

  • Graduate from a college or university accredited by the National League for Nursing Accrediting Commission (NLNAC), or The Commission on Collegiate Nursing Education (CCNE).

  • Possesses one of the following certifications:

  • Commission for Case Manager Certification Certified Case Manager (CCM)

  • Certification of Disability Management Specialists Commission: Certified Disability Management Specialist (CDMS)

  • Association of Rehabilitation Nurses: Certified Rehabilitation Register Nurse (CRRN)

  • American Board for Occupational Health Nurses Certified Occupational Health Nurse (COHN) OR Certified Occupational health Nurse-Specialist (COHN-S)

  • National Board for Certification in Continuity of Care: Advanced Certification in Continuity of Care (ACCC)

  • Commission on Rehabilitation Counselor Certification: Certified Rehabilitation Counselor (CRC)

  • American Nurses Credentialing Center Nurse Case Manager (RN-NCM)

  • National Academy of Certified Care Managers: Care Manager Certified (CMC)

  • One year experience in nursing.

  • Current, full, active, and unrestricted license to practice as a Registered Nurse.

  • Work involves sitting and standing for prolonged periods of time.

  • May require bending, stooping and lifting up to 15 lbs.
