Ecumen Lakeshore consists of 140 independent living and assisted living rental apartment homes as well as 60 short stay rehab beds, 20 additional residences for seniors requiring memory care services and is located in Duluth, Minnesota. Responsibilities: The Nurse Manager, RN is responsible for facilitating the admissions and discharge planning processes for the Nursing Department, coordinating care provided by other members of the health care team, making referrals for follow-up care, and consulting with physicians to provide appropriate care. This position is responsible for supervising, coordinating and coaching staff that deliver nursing care to the clients in accordance with current regulations. Essential job responsibilities include: Coordinates patient admissions, including data collection regarding patient condition at time of admission. Coordinates initial care planning process, including documentation in patient chart and other designated communication areas. May supervises employees in the nursing department, which may include: training and educating employees, planning, assigning and directing work, conducting performance appraisals, delivering praise and corrective action to employees, addressing complaints and problem resolution. Prepares discharge paperwork, coordinates services in preparation for discharge, including pharmacy contact and any other services patient may need post-discharge. Observes resident, records significant conditions, vital signs and reactions, and notifies supervisor or physician of any change in resident's condition, and updates charts to reflect any changes. Coordinates, reviews, and provides notification of patient appropriateness of Medicare Coverage, in accordance with policy/procedures/regulations. Documents Medicare determination of coverage in patient chart. Participates in the Quality Assurance program as delegated by the Director of Nursing. Qualifications: Minimum Required Qualifications: Graduate from an accredited school of nursing. Previous geriatric experience RN license in good standing in the state in which they are employed Current valid driver's license and proof of vehicle insurance (if applicable) Ability to communicate effectively in both written and verbal formats.
