Determines priority of care via personal interaction or telephone and triages appropriately; conducts patient/family/significant other education; provides nursing care and assists physicians; meets the qualifications to provide care for patients in the specific age range on their assigned nursing unit; provides access to appointments.
Works under the supervision of the Clinic Lead Nurse.
MAJOR DUTIES AND RESPONSIBILITIES:
*1. Determines urgency of patient problems and triages appropriately:
*a. Gathers information specific to the current complaint.
*b. Obtains relevant past history.
*c. Assesses urgency of problem based on knowledge of health, cardiovascular illnesses, and pre-established protocols.
*d, Offers information relevant to symptoms based on established protocols.
*e. Confirms understanding of instructions.
*f. Provides appointments as indicated.
*e. Documents interaction and notifies appropriate provider as necessary.
*2. Facilitates prescription orders as needed.
*3. Provides health teaching as needed regarding medication, disease process, preparation for treatments and diagnostic testing. Documents patient education in EPIC.
*4. Responds to inquiry of test results and determines appropriate response.
*5. Acts as informational resource for the services provided by the Cardiology Department.
*6. Rotates out of triage to clinical role as needed.
*1. Performs initial assessment and triage of new patients based on information obtained pertinent to presenting condition.
*2.Reassesses patient's physical status as well as their understanding of their condition at time of return visit.
*3.Determines urgency of problem of walk-in patients not yet seen by a physician based on patient assessment and triages properly.
*4. Evaluates emergency situations involving patients prior to or after physician visit or treatment and triages appropriately.
*5. Completes documentation of patient assessment and care.
*6. Develops plan of care with patient/family/significant other based on individualized need and mutually agreed upon outcome goals identified during assessment and provides nursing care in the following areas:
*a. Obtains vital signs, height, weight, medication list, pain assessment, advanced directive, and allergies at each clinic visit and documents in EMR.
*b. Provides treatments, intravenous infusions and blood transfusions.
*c. Cares for wounds and changes dressings.
*d. Administers medication and vaccines.
*e. Obtains and dispatches laboratory specimens.
*f. Performs specific procedures, nursing care according to clinic policy and patient needs.
g. Obtains necessary referrals and outside test results for patients new to Cardiology.
*7. Conducts patient/family/significant other education appropriate to clinic area and completes documentation; assesses patient's level of understanding and instructs appropriately; does follow-up to ensure understanding.
*8. Evaluates patient care to assess the accomplishment of identified outcome goals and revises plan as needed.
- Assists in the development of educational programs and procedures for clinic area.
*10. Assists physicians with patient examinations, procedures, and diagnostic studies.
*11. Assists patients as needed.
*12. Completes charge documents in the Electronic Medical Record per clinic policy.
*13. Directs other nursing personnel within the clinic; provides input into the
evaluation of those personnel as requested.
*14. Assists with admission of patients from clinic to hospital and provides important
patient information to assigned patient area.
- Assures examining rooms are stocked, clean, and orderly.
*16. Prepares requisitions for supplies and equipment repair.
*17. Assists with the orientation of personnel and students.
Promotes good communications, team work, and positive interpersonal relationships. Utilizes proper reporting channels.
Utilizes appropriate hospital/community referral agencies.
Participates in quality improvement activities and evaluation to ensure quality and appropriateness of patient care, appropriateness of patient education and follow-up, and provision of quality care in a safe, efficient, and timely manner.
*21. Maintains and updates clinical skills and knowledge by attending applicable continuing education courses and/or obtaining certification when applicable.
*22. Participates in nursing service organization activities and committees as requested.
*23. Responsible for actively participating in patient safety improvement and management program. An active participant:
a. Assumes responsibility for identifying processes or systems that could potentially lead to errors and adverse events.
b. Knows and follows organizational and departmental policies and procedures applicable to assigned duties.
c. Participates in required organizational and departmental patient safety education programs and other activities designed to improve departmental and organizational patient safety.
d. Promptly reports serious events and incidents in accordance with established hospital policy and procedure.

reasons for which the employee is qualified to perform.

EDUCATION AND/OR EXPERIENCE:
Registered nurse currently licensed in Pennsylvania or eligible to be licensed required.
Minimum of two years cardiac nursing experience required.
BLS and ACLS certification required.
