Position Description :
Penn Medicine’s Home Care Division is looking to grow and expand our Home Care Resource Team of Registered Nurses.
The Penn Care at Home Resource Team consists of expert and highly skilled nurses who participate in the development and/or implementation of the plan of care for patients receiving services in the home. Penn Care at home serves patients in the following five counties including Bucks, Chester, Delaware, Montgomery and Philadelphia counties.
Nurses in the Resource Team are assigned to patients based on immediate patient care needs and/or in response to organizational staffing needs. As such, these RN’s cover a larger geographical territory and may or may not carry a case load of patients.
The ideal candidate must be flexible, independent and able to shift gears easily while responding to challenging patient assignments on a daily basis. To excel in this role, you must exhibit stellar assessment, critical thinking, and prioritization skills and be able to quickly familiarize yourself with the patients plan and be able to step in and implement the plan of care on behalf of the primary nurse case manager.

Minimum Requirements :
Graduate of an NLN or CCNE approved school of professional nursing.
Licensed to practice professional nursing in the Commonwealth of Pennsylvania.
Minimum of 18 months of work experience as a professional nurse.
Acute care experience highly preferred.
Experience with PICC lines, IV’s and Central lines is preferred.
A BSN from an accredited program is required for all new RNs hired.
Must have Basic Life Support certification, have a car, hold a valid driver’s license, and have car insurance. Responsible for visiting patients in various geographic areas as determined by agency contracts.
