TERRITORY: Lake in the Hills, Algonquin, Arlington Heights area

On-Call once per month

Administrative Office in Barrington.

The Resource Nurse supports the Team Manager by; acting as a leader, role model and subject matter expert for clinical team members; assisting the Team Manager with leadership and development responsibilities; and providing patient centered care and collaborates with interdisciplinary teams for optimum patient/family outcomes. The Resource Nurse will also be a preceptor for onboarding new field team members.

Provides patient care visits for admission, assessment and pain and symptom management and support during the dying process.

Educates current and prospective patients and families regarding hospice and the end of life.

Documents patient visits and phone calls in an accurate and timely manner.

Reviews and updates care plans, medication and equipment orders and completes orders of new medication and supplies for patients.

Monitors visits and calls of certified nursing assistants.

Updates the patient's physician and/or medical director regarding any changes in status.

Attends and participates in interdisciplinary team and RN meetings regarding patients and families and collaborates with the interdisciplinary team appropriately.

Assesses family and caregiver ability to provide safe care, carry out care plan and educate on the proper functioning of all equipment.

Enhances self knowledge regarding new medical and hospice information.

Assists with on-call duties.

Other duties may be assigned or required for the performance of this position.

Required Experience:

Associates degree in nursing

Training in hospice and palliative care and pain and symptom management, required

Registered nurse licensed in the state of Illinois

Certified Hospice and Palliative Nurse preferred

2 to 3 years experience as a registered nurse in the healthcare setting, preferably in hospice

Experience working with patients of all ages

Certain positions require Pediatric skill competencies on an infant, toddler, preschool, school age or adolescent.

Communication skills, verbal and written

Organization skills

Computer skills (Word, Excel, patient database, e-mail), voicemail

Competent in pain and symptom management

Able to use medical equipment

The Resource Nurse assists the Directors with supervision of the CNAs and the orientation of new RNs.

Frequently works under tight deadlines and with interruptions

Frequently sitting and working at the computer

Frequently working with confidential information

Frequently lifting and moving patients. May be required to lift up to 25-50 lbs frequently and move up to 200 lbs occasionally with the assistance of a gait belt or hoyer lift.

Assistance in administering medications that are ordinarily self-administered, when indicated.

Frequently driving to complete patient visits
